I hereby give permission for my child to participate in Vacation Bible School at Calvary Baptist Church, Sandusky, OH. I understand that if my child does not behave in an appropriate manner, I may be contacted to come pick up my child and remove them from the church property. I hereby release and hold harmless Calvary Baptist Church of Sandusky, OH, including it's leaders and workers, with respect to any and all illness, injury, disability, death, or loss of damage to person or property whether caused by negligence or otherwise. 

Calvary Baptist Church may use photographs of participants on our Facebook page and Website. We will not post the names of any children. Authorization is assumed. If you wish to withold permission, please contact the church office at 419.625.9690.
